If you've created an account on www.healthcare.gov, bring your username, password and PIN
Information for all individuals in your household (those listed on your tax return)
Name and date of birth. Address – bring a driver’s license or PG&E/phone/cable bill if you can
Current income – bring a paystub or current tax return
Social Security number for those individuals who are applying for coverage – bring green card or visa if you have one
